Officials Vaccination Status
All Junior and Senior hockey leagues in Alberta, as well as ACAC and USports, have implemented the Restriction Exemption Program (REP) in order to play hockey this season in accordance with Government of Alberta mandates. As well, many facilities/ arenas/ municipalities in the province have implemented REP. Under the REP, on-ice officials are required to provide valid:
- proof of vaccination, or
- proof of a privately-paid negative rapid test result taken within 72 hours of service, or
- documentation of a medical exemption
Rather than have every league ask you individually about your vaccination status, to allow games to be assigned to appropriate individuals, and to avoid cancelling hockey games, Hockey Alberta are asked to do the following:

2. Fill out the form below. Upload a copy of Government of Alberta approved Vaccination documentation, and upload a copy of the Consent to Gather Medical Information form.
Officials Vaccine Declaration
This form is for officials who skate games at the Junior, Senior, ACAC and USports levels. Completing this form is required for assigning of games at any of these levels.
